UK's first eyelash transplant using hair from the head [ Trichotillomania ]

Cosmetic surgeons have performed the UK’s first eyelash transplant using hair taken from the back of the head.

The teenage patient had been suffered from trichotillomania, sometimes also known as trichotillosis or ‘trich’, an impulse control disorder where the patient repeatedly pulls out an area of hair. In this particular case, the patient had pulled out most of her upper eyelashes, having suffered from the disorder for the last 17 years.

The four hour cosmetic procedure involved taking hair from the back of the patient’s head and then grafting individual hairs in the eyelid to replace the lost eyelashes. Over the next four to six months, the lashes should start to thicken up. The eyelash replacement treatment cost around £3,500.

Trichotillomania often begins when patients are teenagers and can lead to depression brought on by the bald patches. Hair grafting offers a way for sufferers to look ‘normal’ again and allow the bald patches to regrow again.

Trichotillomania is a compulsive urge to pull and/or twist of the hair until it breaks off, resulting in noticeable hair loss.  The hair may be lost in round patches or diffusely across the scalp.  It is a type of compulsive behaviour and its causes are not clearly understood, although symptoms usually begin before the age of 17, with women almost four times likely to be affected than men.  It may affect as much as 4% of the population.  The action of pulling on the hair over a long period of time results in the gradual shrinking of the hair follicle and so the hairs produced become finer and finer and permanently damaged if not treated.

Some hair loss is the result of a type of immune disorder known as alopecia areata: some 2.5 million people suffer from this condition in which antibodies attack the hair follicle, causing the hair to fall out. Can anything be done for female pattern hair loss?

QUESTION:  Can anything be done for female pattern hair loss?

 ANSWER:  Yes – treatments similar to those for men.

As in men, there are many causes of hair loss in women – including scalp infections and inflammation, and auto-immune attacks on the follicles in the skin that make hair.

But the most common problem for women, especially after menopause, is “female pattern hair loss,’’ or androgenetic alopecia. High levels of the male hormone testosterone can trigger this, but “most women with androgenetic alopecia have normal levels of testosterone,’’ says Dr. Lynne Goldberg, a dermatologist and director of the hair clinic at Boston Medical Center.

These women may have hair follicles that are genetically programmed to be extra sensitive to testosterone, which causes them to produce thinner – and eventually, no – hair, says Dr. Deborah Scott, a dermatologist at Brigham and Women’s Hospital.

The first treatment, Scott says, is Rogaine (minoxidil), which must be used continuously to sustain any benefit. If it does not make new hair grow, it can help slow hair loss.

Propecia (finasteride) is used to treat hair loss in women in Europe, but is not government-approved here for that purpose. Studies suggest it does not work well in women, Scott says, though some doctors prescribe it, which they are allowed to do. Another option, though it, too, isn’t approved for this use, is Aldactone (spironolactone), a diuretic whose side effect is to block testosterone receptors.

Hair transplants are a good, but expensive, option, and not covered by insurance. The latest technique uses “micrografts,’’ meaning that hair follicles are surgically removed from one spot on the head – usually the back of the scalp – and inserted one or two hairs at a time in the bald area. While hair follicles in the crown of the head are often destroyed by testosterone, those at the back are not.

However, if your underlying problem is inflammation or infection, transplantation won’t help.

Hair Loss Cases Among Older Women Triple in 7 Years

Hair loss, once considered a problem only for middle-aged men, is now affecting a growing number of women in their 50s. According to a report on medical expenses released by the National Health Insurance Corporation on Sunday, the number of female hair-loss patients aged 50 years or older tripled from 5,713 in 2001 to 16,624 in 2008. The figure has increased steadily by 16.5 percent a year on average since 2001.

The total number of hair-loss patients climbed 60 percent from 103,000 in 2001 to 165,000 in 2008. Male patients totaled 85,000 last year, outnumbering female patients by 5,000, showing that the problem still afflicts more men than women.

“There are diverse causes for hair loss including hormonal imbalance, nutritional deficiency, side effects of drugs such as contraceptives, and physical and mental stress,” said Cho Nam-joon, a professor of NHIC Ilsan hospital. “Now more people are exposed to these causes and thus hair-loss patients are increasing.”

The growing number of hair-loss cases among older women is the result of not only hormonal changes resulting from menopause but also stress and fatigue, Cho said. He recommended that women experiencing hair loss eat a balanced diet and avoid stress.

Thyroid problems could and do cause hair loss

“I went to my doctor for hair loss. The doctor said I had a thyroid problem and gave me a prescription for methimazole. The hair thinning continued, and I read that this medicine can cause hair loss. I have stopped taking it. What can I do? I am 50 and a woman.” – G.J.

ANSWER: On average, people lose 100 hairs a day, and 200 on shampoo days. To be certain you are exceeding those numbers, collect the hair from your comb and brush every day and do so for two weeks to see what your count is. That will tell you if you truly have a problem.

Thyroid illnesses can cause hair loss. Your medicine was for an overactive thyroid gland. If there is solid laboratory evidence that your gland is putting out too much thyroid hormone, you had better go back on the medicine or seek an alternative treatment for it. An overactive gland can cause troubles greater than hair loss. Furthermore, although the medicine can cause hair loss, you might not have given it a long enough chance to work.

Stressful situations lead to hair loss. After having a baby, many women experience a tremendous loss of hair. The same can happen to people who have had an illness, particularly ones that cause fever. Such hair loss almost always reverses, given time.

One of the most common causes for female hair loss is the effect of male hormone on the hair follicle. Women do make male hormones. After menopause, when there is a drop in female hormone production, there is a relative increase in the amount of male hormone in a woman’s body. Just as it does in men, it can make hair follicles wither and die.
